Cowlitz Volleyball Club - 2007 Season
Tryout Information

Players, just a friendly reminder that the informational meeting and tryouts are just around the corner.

The Informational meeting for Cowlitz Volleyball Club is on Wednesday November 8^th at Monticello Middle School.

12-U & 14-U 6:30 pm
(parent or guardian must accompany each athlete)

16-U &18-U 8:00 pm

Due to the high number of athletes trying out this year and to avoid long lines the day of tryouts please come prepared to pay the 20$ tryout fee the night of the informational meeting.  The tryout fee covers insurance for the day of tryouts and also includes a tryout t-shirt.

This year Columbia Empire Volleyball Association (CEVA) is requiring that all players register online before trying out. You can access online registration on the CEVA web site at columbiaempirevb.com.  If you have questions with online registration you can contact Shannon by email at shaye4745@adelphia.net or by phone at 431-7404.  There are very good instructions on the CEVA website for navigating the registration system. 

After successful registration the parent and player will need to print and sign the “waiver & code of conduct” and “player medical history form” Note: you will have to fill out the information requested on this form, it is 2 pages.  You will need to bring these forms to the information meeting or tryouts.

• 2 PAGE “PLAYER MEDICAL HISTORY” FORM COMPLETED AND SIGNED
